The home care package (HCP) scheme is intended to help older Australians live a safe, healthy and happy life in their own homes. HCP funding can be used to provide assistance and medical care in the home as well as equipment and home safety modifications.

Healthcore therapists can assist by discussing your needs and goals with you and also by conducting a detailed assessment of your home. They will liaise with your HCP provider to ensure that all recommended interventions are implemented as soon as possible.
